Leg Ulcer Hosiery Kits

clinical information

A first line treatment for venous leg ulcers.

Leg Ulcer Hosiery Kits are an alternative to compression bandaging, delivering full therapeutic compression. They support patient self care, reduces nursing time to care, and eliminates clinical variation in application technique.

Leg ulcer hosiery kits can be used as first-line treatment of venous leg ulcers (VLUs) for patients who have low to moderate wound exudate and minimal limb distortion.

Benefits include:

  • Optimal 40mmHg compression for effective VLU healing

  • Simple to apply consistently and safely

  • Requires minimal training and can be reapplied by the patient, carer and other members of the multi-disciplinary team

Leg Ulcer Hosiery Kits available:

  • Activa Leg Ulcer Hosiery Kit - for the treatment of limbs without oedema

  • ActiLymph Leg Ulcer Hosiery Kit - for the treatment of limbs with oedema

What's contained within a Leg Ulcer Hosiery Kit:

Activa Leg Ulcer Hosiery Kit

  • x2 10mmHg liner

  • Open-toed British Standard Class 3 Stocking (25-35mmHg)

ActiLymph Leg Ulcer Hosiery Kit

  • x2 10mmHg liner

  • Open-toed European Classification Class 2 Stocking (23-32mmHg)

The two garments are worn together, liner followed by the open-toe compression stocking, to provide full therapeutic compression.

Activa® Leg Ulcer Hosiery Kit, ActiLymph® Hosiery Kit and Activa® 10mmHg Liners can be found under ‘Venous Ulcer Compression System’ in the Drug Tariff and are available on FP10/GP10

Please note - Not everyone with a leg ulcer is suitable for the hosiery kit and some people may need to wear bandages depending on the size and shape of the leg and ulcer.

Leg Ulcer Hosiery Kits Indications & Contraindications

Indications

Indications

  • Compression of limbs with venous disorders
  • Treatment of venous leg ulcers
  • Prevention of recurrence of venous leg ulcers and severe varicose veins
  • Ideally suitable for patients who will not accept compression bandages e.g. cosmetic reasons in younger patients

Contraindications

Contraindications

  • This system may be unsuitable where there is heavy exudate and for large wounds
  • For large or irregular shaped limbs it may be more appropriate to use compression bandages
  • Padding and bandages are recommended for patients with very small limbs or with bony prominences
  • Lymphoedematous limbs should be treated with short stretch bandages e.g. Actico® prior to going into the leg ulcer hosiery kit
  • Diabetic patients - unless after specialist referral and under strict supervision
  • Arterial insufficiency/arterial disease/ischaemia according to vascular assessment.
  • ABPI of <0.8 unless after specialist referral and under strict supervision and regular follow up
  • Congestive cardiac failure as compression could lead to cardiac overload
  • Peripheral neuropathy
  • Rheumatoid arthritis
  • Known sensitivity to the fabric of the stockings

Warnings

Warnings

  • For large or irregular shaped limbs, or where there is heavy exudate, it may be more appropriate to use inelastic cohesive compression bandages

  • Padding and bandages are recommended for patients with very small limbs or with bony prominences

  • Diabetic or rheumatoid arthritis patients - unless after specialist referral and under strict supervision

An audit of the use of compression hosiery kits as first line treatment of patients with leg ulceration in leg ulcer clinics

In an effort to reduce waiting times it was decided to increase the number of patients using hosiery kits as first line treatment for their leg ulcer. An audit showed this approach reduced referral waiting times and time to healing.

Clinical and cost-eff ectiveness of compression hosiery versus compression bandages in treatment of venous leg ulcers (Venous leg Ulcer Study IV, VenUS IV): a randomised controlled trial

Drawbacks exist with the standard treatment (four-layer compression bandages) for venous leg ulcers. We have therefore compared the clinical eff ectiveness and cost-eff ectiveness of two-layer compression hosiery with the four-layer bandage for the treatment of such ulcers.

The leg ulceration pathway: impact of implementation

The implementation of a pathway for the treatment of leg ulceration which aids diagnosis and uses compression hosiery kits as a first line has been shown in audits to improve the patient experience, release nursing time and ensure effectiveness of care.
